System Components

1. Hardware (PCs or PDAs used to administer surveys, to process and store data, and printers to produce reports).

2. Software that provides for questionnaire administration, data processing, data base management, report generation, and various other administrative functions.

3. Questionnaires:

• Patient Intake: Baseline Patient Questionnaire that screens for depression and indicates whether referral or treatment is appropriate.
• Patient Update: Completed concurrently with treatment the Update information shows change in the severity of depressive symptoms.

4. Procedures for administering the patient questionnaires that include a manual describing how to install and use the system.

5. Reports that provide information for clinical decision support (individual patient reports) and for outcomes assessment (program level aggregate data).
