What Polaris-CV Measures

Polaris-CV is designed to assess:

(1) Depression, anxiety, and other psychosocial risk factors

(2) Behavioral and clinical history risk factors

(3) Level of concern regarding depression

It collects, stores and reports information relating to:

• Personal and family history of depression

• Personal and family history of cardiac problems

• Depression: severity of symptoms and change over time

• Anxiety: severity of symptoms and change over time

• Somatic symptoms of depression

• Psychosocial risk factors (e.g., hostility, anger, will to live, social support)

• Behavioral risk factors (e.g., smoking, substance abuse, lifestyle)

• Patient concern about feelings of depression
